Fees and Insurance
Rates
$100 for payment at the time of service ($120 with a $20 discount)
Sliding Scale Fee
Our sliding scale fee is a reduced fee based on the size of your family and gross annual income, it is available on a limited basis, from $55-$100.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health benefits?
- What is my deductible for out-of-network service and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider? Some networks include Karen for her Licensed Massage Practitioner services, but not for her services as a Licensed Mental Health Counselor. Please clarify these coverage issues with your insurance provider.
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Where do I send the receipt for my session for reimbursement?
Karen is an in-network provider for Uniform Medical Plan.
If you are covered by another plan, you still may be able to receive covered treatment.
For plans other than Uniform Medical Plan, Karen is considered an out-of network therapist, which means that she is not contracted with them as a Licensed Mental Health Counselor. What this means for you is that you pay in full at the time of service and that you may get reimbursed by your insurance company for a portion or all of the payments made for counseling. This is to be worked out between you and your insurance company.
The benefit of using an out-of-network therapist is that you have more control over your counseling needs and more control over what information is or is not shared with your insurance company about your treatment.
Payment
Cash, or check due at the time of service
Cancelation Policy
When you cancel within 48 HOURS of your appointment START TIME you will be charged $100 or a previously agreed upon amount. This charge can not be billed to any insurance company and is due within a week of your cancellation or at your next appointment.
